When you are living with a chronic illness, we understand that you have many medical, family, and other concerns that may need attention and balance.

If you have been in the hospital or emergency room within the last year with heart problems, stroke or other chronic illness that is affecting your daily life, you may qualify for our Acute Care Management Program.

Through this program, our nurses and social workers will work with you and your doctor as a team to create a plan that will simplify your care.
