Substance abuse and mental health disorders can significantly affect your overall health, your ability to work, and your relationships. The Behavioral Health Care Management program is offered to those who might benefit from follow-up care for mental health, drug and/or alcohol problems.

If you have recently been to the hospital with psychiatric problems or have had another serious behavioral health situation, you may qualify for this program.
