Controlling your blood sugar may mean fewer eye, kidney, nerve, and heart problems in the future – problems that often affect people with diabetes.

Through our Take Control of Diabetes program, you will receive valuable diabetes-related health information and be partnered with a nurse to work with you and your doctor to manage your diabetes. You can talk with the nurse to get new meal ideas and help with your medications, reminders for routine screening exams, and ideas for taking better care of yourself. You also may discuss the complications that can occur if your blood sugar
levels aren’t properly controlled.
