Behavioral Health Care Management Program
Questions and Answers
Q: What is the Behavioral Health Care Management Program?
A: The Behavioral Health Care Management Program is offered to Preferred Care members who might benefit from having some help with follow-up care for mental health or drug and alcohol problems. This service is free, it is not treatment, and it is conducted primarily by telephone. Having a psychiatric hospitalization or other serious behavioral health situation may qualify you for the program.
Q: Why would I want or need care management?
A: With care management you have someone who can help you remember to keep your appointments, support you in asking questions of your doctor or therapist, help you find other support services you may want or need, or even provide you with educational materials about different medical or behavioral health problems.
Q: Who are the staff of the Behavioral Health Care Management Program?
A: All of the Behavioral Health Care Management staff are licensed social workers and/or registered nurses with many years of experience. Although they will not be providing treatment, they are very familiar with the providers in our community.
Q: If I qualify for care management, can I refuse it?
A: You can refuse care management, but this free service might help you deal with what are sometimes complex or confusing situations. The care manager wants to help you in situations where a little extra support might make the difference.
Q: What happens when I am enrolled in the program?
A: When you are enrolled in care management, a care manager will be assigned to work with you. They will send you a letter of introduction and then call you to see how they can help.
Q: Why would Preferred Care offer this service for free?
A: Preferred Care believes that all of our members should receive quality care. We also believe that if you receive some help in following through with that care, you are more likely to feel better. That helps Preferred Care, too, because getting the right help at the right time can help keep larger, more costly problems from occurring or reoccurring.
Q: If I have other questions about the program, including whether or not I qualify for it, who do I talk to?
A: If you have questions about the program, please call Preferred Care's Behavioral Health Unit at (585) 327-2477 or (800) 933-3920, x2477.
