Support for Patients
Working in partnership with you, Preferred Care's team of nurses, therapists and care managers provide patient support that includes:
- Accessing medical services
- Exploring mechanisms to enhance self-care practices through educational mailings, newsletters, phone calls or home visits (when applicable)
- Accessing useful community programs
- Locating free wellness classes and services
There is no charge to your patients for most of these programs.
Provider Support
Preferred Care's care management programs provide you with easy, effective ways to enhance your patients' care.
- Our programs have documented improved patient outcomes
- Providers can choose to receive urgent notifications based on set parameters for weight and other symptom changes they set for patients. Urgent reports or periodic telephonic contacts provide pertinent clinical updates.
