Take Control of Diabetessm
Diabetes Disease Management Program

The Preferred Care Take Control of Diabetes program is designed for any member with diabetes age eighteen (18) years and older and is focused on patients who, in addition to diabetes, have complicating co-morbid conditions and management challenges. Goals of the program include:

  • Improved patient self-management skills.
  • Increased awareness of preventive measures through education.
  • Collaboration with providers to support their plan of care.

The Take Control of Diabetes program provides each enrolled member with a case manager, who may be a registered nurse, a CCM-certified registered nurse or a CDE-certified registered nurse, as part of a multi-disciplinary team working to treat members with diabetes. Members can contact their case manager any time they note a change in their condition or require assistance, even after participation in the program is completed.

The primary goal of the Take Control of Diabetes program is to help members become informed, self-empowered and as active as possible. This is accomplished through setting collaborative goals, care coordination, and providing positive feedback for positive healthy behaviors. Case managers provide interventions that may include:

  • Helping to monitor symptoms.
  • Improving behaviors necessary for successful diabetes management: symptom monitoring, treatment adherence, and self-management .
  • Enhancing communication and care coordination between members and health care providers.
  • Working with members and/or their primary caregivers to assure that education is being directed to those responsible for the member’s care.
  • Increasing the member’s knowledge, through education and symptom monitoring, for more effective use of medications.
  • Assessing functionality and compliance with treatment plan.
  • Evaluating the home environment.
  • Aiding in the management of co-morbid conditions.
  • Promoting lifestyle modifications related to smoking cessation and other healthy choices.

As part of the coordination of care, providers are advised of member enrollment and program completion, as well as any significant clinical events and/or status changes.

Providers are eligible for reimbursement upon completion of an enrollment form for each patient who:

  • Meets program telephonic management criteria; and
  • Successfully enrolls in the program.
