System and Association Affiliation

Preferred Care offers physicians and other health care providers a choice in how they work with us. These contracting options are described below. Many physicians have chosen to work through a health system or an association of providers. A description of these provider associations currently affiliated with Preferred Care follows.

Direct Contract Providers (D) Direct contract providers are those providers who contract directly with Preferred Care.

GRIPA (G) Greater Rochester Independent Practice Association
GRIPA is an integrated health care delivery network of more than 280 primary care physicians (PCPs) and over 475 specialists, as well as allied and ancillary health care professionals. GRIPA also includes both the Continuing Care Network, Inc., which provides nursing home, home health, and other long-term care services, and the Behavioral Health Network, for mental health and substance abuse services. GRIPA provides physicians the opportunity for greater participation in clinical decision-making and care management for their patients, including those in Preferred Care and other managed-care initiatives. GRIPA ensures high-quality coordinated care for patients in need of a full spectrum of services, from physician care through hospital, ambulatory, nursing home, home health care and mental health care services. Lifetime Health (L)
Lifetime Health is a mixed model delivery system with a network of primary care physicians located in the greater Rochester area. Its staff of affiliated physicians strive to continually improve the quality of health care services delivered to the Greater Rochester Area community. In pursuing that objective, Lifetime Health works collaboratively with Preferred Care to address the health-related issues that are most important to community members in Lifetime Health's service area.

Preferred Health Network (P)
Preferred Health Network (PHN), formerly known as Rochester Community Individual Practice Association IPA #1, is a network of more than 700 primary care physicians and approximately 1,000 specialists providing you with comprehensive medical care. Preferred Health Network, which operates under the philosophy that physicians are the most appropriate managers of care, is committed to physician and patient advocacy and to the delivery of high-quality, cost-effective medical services.
