Prior Authorization of Certain Prescription Drugs
At Preferred Care our top concern is protecting your health. Through the Prior Authorization process, we can work with your physician to help make sure that you receive quality care and let you know up-front that your treatment is covered. If your doctor prescribes a medication that needs prior authorization, he/she will need to get approval from Preferred Care before you can begin treatment. If you don’t get approval, Preferred Care may not cover the drug.

In some cases, Preferred Care requires you to first try certain drugs to treat your medical condition before we will cover another drug for that condition. If you meet these requirements, you will be able to get your prescription without prior authorization.
Quantity Limits
This means that there is a limit to the amount of a prescription drug that you can purchase per month without requiring prior authorization. If you need more than the limit allows,
your doctor must request a prior authorization.
All drugs prescribed for off-label use require prior authorization. Prior authorizations should be faxed by your physician to the Preferred Care Pharmacy Department at Fax: (585) 258-8621.
