Basix...Great Coverage at Great Savings

Preferred Care Basix is one of our lowest-cost plans. It offers you the same high-quality member service you have come to expect from all Preferred Care plans, without costly features you may not need.

This plan provides members with basic benefits and access to Preferred Care’s full network of doctors and health care professionals. Coverage for the care you need most Preferred Care Basix gives you the coverage you need, with a focus on preventive care:

• no copays for well-baby and well-child check-ups to age 19;
• no copays for childhood immunizations and vaccinations;
• low copays for annual adult physical exams;
• low copays for mammograms;
• low copays for annual gynecological exams;
• low copays for x-rays, lab services, or other diagnostic tests;
• coverage for eye wear;
• coverage for dependent children to age 19; and
• coverage for dependent children to age 23, provided they are enrolled in an accredited educational institution.

Large provider network

As a Preferred Care Basix member, you have access to the full Preferred Care network of more than 4,500 health care professionals and our national provider network of doctors, hospitals, therapists, labs, and other health care professionals.

Preferred Care HealthDollarssm

When you enroll in Preferred Care Basix, you are eligible for $50 in HealthDollars to spend during the year on any health, wellness, or fitness program.
