To be eligible for Preferred Care GoldAnywhere:

• You must be eligible for Medicare Part A and enrolled in Medicare Part B.
• You must have a permanent residence in Preferred Care’s service area which includes Genesee, Livingston, Monroe, Ontario, Orleans, Seneca, Steuben, Wayne, Wyoming, and Yates Counties.
• If you have end-stage renal disease, you
  must have had a successful kidney  transplant OR you must be currently covered by Preferred Care.
