The choice is yours!

Preferred Care TriVantage – our most popular health plan – is a unique plan that lets you choose the benefit package that best fits your lifestyle. You get to choose from three benefit
options – Active Lifestyles, Family Focus, or Healthy Alternatives. The difference between the options is the savings on the services you value most.


With Preferred Care TriVantage you get:

  • coverage that fits how you live, so you pay less for the services you use the most;
  • options for alternative therapy coverage, maternity coverage, and low-copay eye
    exams; and
  • added features for healthy activities beyond typical health care.

Active Lifestyles is ideal for people interested in staying healthy and active, and offers:

  • very low copays for visits to primary care physicians (PCPs) and gynecologists;
  • emergency care;
  • routine eye exams;
  • up to a $300 credit toward a membership at any health and fitness club; and
    special benefits for massage therapy, acupuncture, and routine chiropractic care.
  • TriVantage subscribers also are eligible for $50 in HealthDollars to spend on during the year on health, wellness, and fitness programs at any location nationwide!

Family Focus helps you care for children and keep the whole family healthier, and offers:

  • fully covered maternity care;
  • low copays for adult primary care physician (PCP) visits;
  • low or no copays for sick-child doctor visits;
  • low or no copays for PCP well-child visits through age 18; and
  • $300 toward Children’s Swimming Lessons, Driver’s Education Training,
    or Lifeguard Training for dependent children.

Healthy Alternatives is for people with the freedom to travel or the desire to seek out alternative therapies, and offers:

  • out-of-network coverage at 75% for care without a referral, up to $25,000 after a
    $250 deductible; and
  • special benefits for massage therapy, acupuncture, and routine chiropractic care.
