Understanding your Eye Care and Eye Wear Benefits

Healthy eyes are an important part of living well, which is why Preferred Care makes taking care of your vision simple and easy.

Eye Care Benefits

  • No referral is required for routine eye examinations.
  • Routine eye exams are covered once each calendar year (unless otherwise specified in your particular health plan).

Commercial HMO/PPO

Members with eye wear coverage are eligible for a discount when purchasing eye wear (lenses, frames, eyeglasses or daily wear contact lenses) at participating providers.

Eyewear provider gives 20% discount off
regular retail price.
A $60 credit is available if your employer has purchased eyewear coverage as part of your plan. (Any unused portion of this benefit cannot be carried over from one calendar year to the next.)

Example:
$300 retail eyeglass cost minus 20% discount, equals
$240 minus $60 credit, equals
$180 cost to you

Gold / GoldAnywhere
Eyewear provider gives 20% discount off
regular retail price.
Preferred Care will pay for the first $100 on eye wear, contacts, frames, and lenses per calendar year.
Any unused portion of this benefit cannot carry over from one calendar year to the next.
You pay 100% of the cost over $100.

Example:
$300 retail eyeglass cost minus 20% discount, equals
$240 minus $100 credit, equals
$140 cost to you

Option
Option plan members receive full coverage from a specified selection of eye wear.
CareFund HRA

Members with eye wear coverage are eligible for a discount when purchasing eye wear (lenses, frames, eyeglasses or daily wear contact lenses) at participating providers.

Eyewear provider gives 20% discount off
regular retail price.
A $60 credit is available if your employer has purchased eyewear coverage as part of your plan. (Any unused portion of this benefit cannot be carried over from one calendar year to the next.)

CareFund HSA
Contact lenses and eyeglasses are qualified medical expenses that can be reimbursed through your Health Savings Account (HSA) according to IRS guidelines.
Members with no eye wear coverage
Most of our participating eyewear providers offer a 20% courtesy discount off the regular retail price on eye wear purchases (including sunglasses and other supplies).
Just present your membership card.
